Tip
You can't cheat this by replaying only the last chapter on Hard: every chapter has to be completed on Hard. The cleanest route is a dedicated Hard playthrough from the start. Combat rewards patience — use cover, pick off enemies with headshots, and lean on knife takedowns to save ammo.
